The following is a sample pet service agreement

HEHpaws® (“HEH”) agrees to provide dog walking services for your pet(s). The dog walking services will be performed ____________________. The total number of visits will be ____________________ each week.

You will be charged $__________ per walk unless otherwise specified. Payment for the dog walking services is due on delivery of your pet or based on a regular weekly contract. So as to not be charged, all cancellation must be made twenty-four (24) hours in advance and must be acknowledged by HEH.

Services will be performed in accordance with the instructions contained herein. The client waives any claims against HEH except those due to gross negligence or wilful misconduct, and releases HEH from liability, including for injury to or death of a pet, so long as HEH was performing its obligations in good faith in accordance with the Agreement.
